The Rehabilitation Discharge Team is a community-based
team committed to offering a comprehensive rehabilitation service in the home, for up to four weeks
after discharge from hospital. Our clients live in the Hornsby or Ku-ring-gai area and are usually over the age of 50 years.
Who are we?
We are the Rehabilitation Discharge Team, part of the Rehabilitation & Aged Care Service at Hornsby Ku-ring-gai Health. We are a multi disciplinary
team, based at Hornsby Ku-ring-gai Hospital.
What do we do?
We provide regular home visits by the team based on an individualised rehabilitation program. We offer nursing, physiotherapy and occupational therapy,
extending care post discharge from hospital. This service also enables you to be discharged from hospital earlier, as your health care is continued at home.
How you can access our service
Referral is usually made through hospital staff whilst you are in Hornsby Ku-ring-gai Hospital.
You will be seen on the ward prior to discharge by the team coordinator and given information
about the team.
In some cases a local doctor will contact the team to refer a patient living at home, in order to prevent hospital admission.

We are based on the campus of the Hornsby Ku-ring-gai Hospital.
What are our hours?
We operate six days a week and are available by phone from 8.00am – 4.30pm, Monday to
Friday; 8.00am – 12.30pm on Saturday.
Visits to your home by the team will take place between 9.00am -1pm, Monday to Friday and
between 9.00am and 12.30pm on a Saturday.
If we are unable to take your telephone call, please leave a message on the phone – the call will be returned.
