The Aged Care Liaison Nurse (ACLN) is a full time registered nurse responsible for the co-ordination of the process of nursing home placement for inpatients and the assessment of inpatients eligible for hostel care. The role also involves working in the acute wards to comprehensively assess elderly patients with complex care needs; and liaising with the patients, relatives and health care providers in hospital and community settings.

Who am I?

The Aged Care Liaison Nurse (ACLN) service at Hornsby Ku-ring-gai Hospital.

What do I do?

The Aged Care Liaison Nurse is responsible for the co-ordination of the process of nursing home placement for inpatients and the assessment of patients eligible for hostel care. The role also involves working in the acute wards to comprehensively assess elderly patients with complex care needs; and liaising with the patients, relatives and health care providers in hospital and community settings. At weekends the role includes assessment and discharge planning for patients, usually over the age of 65 years, admitted to the Emergency Department, Emergency Medical Unit or other wards.

How you can access our service

No referral is needed. However this Service is only available for clients who are inpatients of Hornsby Ku-ring-gai Hospital.

If you or a family member (currently in Hornsby Ku-ring-gai Hospital) need the help of the Aged Care Liaison Nurse she can be paged from within the Hospital on 52682. The switchboard number, for people calling from outside the Hospital, is 9477 9123 - the switchboard can then page the Aged Care Liaison Nurse for you.

If your family member is not currently in Hospital, the Aged Care Assessment Team would be the appropriate contact for placement issues.

Where you can find me

The service is based on the campus of the Hornsby Ku-ring-gai Hospital. However, the Aged Care Liaison Nurse is usually out in the Hospital wards, so that contact needs to be made in the first instance by telephone or paging.

What are the hours?

Tuesday to Saturday, 8am to 4.30pm.
