Ryde Community Mental Health ServiceRehabilitation ProgrammeCREATEThe CREATE program - Community Recreation, Education Access, Training & Employment - is a specialised & truly multi-disciplinary (clinicians, teachers, trainers) mental health rehabilitation service that provides 3 specific strands of service delivery:
- Community Recreation - is primarily run out of the `HUT' on the Macquarie hospital campus - a social and leisure club which is consumer driven.
- Education Access - an experienced educator coordinates and promotes pathways & partnerships in education and training programmes.
- Training & Employment - training and employment opportunities are provided through the business enterprises of a `not for profit' organisation - Macquarie Area Rehabilitation Specialties Inc. MARS Inc.
The team is based above Cornucopia Café on the old Gladesville Hospital campus & operates during business hours. The core objective of this program is to access, promote & partner opportunities for consumers in these strands: - The award winning Vocational strand is a unique collaboration with a `not for profit' organisation MARS Inc. Supported employment is provided through competitive, commercially viable and successful businesses.
- NSH Area Mental Health is facilitating an integration of all vocational programs across the area and promoting truly Recovery oriented pathways for consumers.
- The CREATE program gives consumers the opportunity to work for award rates of pay in a supportive environment through accredited vocational skills courses. Consumers report improved quality of life and clearly exhibit positive vocational, psychological, social and functional improvement.
`Work is the major social mechanism through which we define who we are, where we belong and how we contribute to society.' `Successful community mental health rehabilitation requires CREATive solutions to turn dreams into realities for consumers.' Community Rehabilitation Team (CRT)The CRT is a large multi-disciplinary team that provides individual clinical rehabilitation interventions including intensive case management for people with serious mental disorders. Interventions are provided predominantly in people's own homes & in the service's supported accommodation residences. The team operates between the hours of 8.00 am to 10.30 pm 7 days / week. The core objective is to provide a comprehensive & timely Rehabilitation Service, that is guided by the principles of Recovery, skill development, health promotion and relapse prevention. The CRT has developed a tiered approach to care & intervention that most directly addresses the Rehabilitation / Recovery needs of individuals referred to this Service. The CRT provides a system of assessment and review of Rehabilitation / Consumer Goals and matches interventions that promote Recovery and independence. The CRT provides service in two streams; the "Support Team" which provides ongoing treatment and support to consumers (disability support), and the "Assert Team" which coordinates all assessments & offers consumers individualised programs and interventions (clinical rehabilitation). Recovery Services The Recovery Service aims to:
- Develop meaningful engagement with service users, provide evidence-based interventions and promote recovery.
- Increase stability within the service user's lives, facilitate personal growth and provide opportunities for personal fulfilment.
- Provide a service that is sensitive and responsive to service user's cultural, spiritual and gender needs.
- Support the service user and his/her family/carers as required.
- Promote effective interagency working.
- Ensure effective risk assessment and management.
Service Principles of Care - A self-contained team responsible for providing the full range of interventions.

- Majority of services delivered in community.
